MAJOR GENERAL (DR.) DONALD J. BUTZ

Major General (Dr.) Donald J. Butz is assistant surgeon general for dental services, Office of the Surgeon General, Headquarters U.S. Air Force, Bolling Air Force Base, D.C.

General Butz was born in Nevada City, Calif., in 1933, and graduated from Christian Brothers School, Sacramento, Calif., in 1951. He received an associate of arts degree from Yuba College in 1953 and then attended Chico State College and the University of California to complete his undergraduate pre-dental requirements. He earned a doctor of dental surgery degree from St. Louis University School of Dentistry in 1959.

The general entered the Air Force with a direct commission as a first lieutenant immediately after graduating from dental school. After completing the basic orientation course at the Air Force Medical Service School, Gunter Air Force Base, Ala., he served as a general dentist and prosthodontic officer with the 6038th USAF Hospital, Misawa Air Base, Japan, from October 1959 to October 1962. General Butz then returned to the United States and was assigned to the 392nd Aerospace Medical Group at Vandenberg Air Force Base, Calif., where he served as prosthodontic officer. He received his regular commission in 1965. The general transferred to the 2795th USAF Hospital, Robins Air Force Base, Ga., in July 1965 and served as prosthodontic officer and assistant base dental surgeon. In July 1968 he was assigned to the Department of Dentistry, Air Force School of Health Care Sciences, Sheppard Air Force Base, Texas, where he served as instructor and course supervisor.

From July 1973 to July 1978 he was assigned to the Medical Plans and Health Programs Division, Directorate of Medical Plans and Resources, Office of the Surgeon General, where he served as dental staff officer and, later, as chief of the Personnel and Education Plans Branch. He then was assigned as the assistant for dental services, Office of the Command Surgeon, Headquarters U.S. Air Forces in Europe, Ramstein Air Base, West Germany. He returned to the United States in October 1979 and served as deputy assistant surgeon general for dental services, Office of the Surgeon General. General Butz was assigned as command dental surgeon, Headquarters Tactical Air Command, Langley Air Force Base, Va., in October 1982. He assumed his present position in March 1987.

His military awards and decorations include the Legion of Merit with oak leaf cluster, Meritorious Service Medal with oak leaf cluster, Air Force Commendation Medal, Air Force Outstanding Unit Award, Air Force Organizational Excellence Award and National Defense Service Medal.

General Butz is a member, as well as a delegate, of the American Dental Association and the American Association of Dental Schools Council of Deans, a member of the Air Force Association, the Association of Military Surgeons of the United States, and the Delta Sigma Delta dental fraternity. He is a fellow of the American College of Dentists. He was the first Air Force dental officer to attend the Interagency Institute for Federal Health Care Executives at the University of Chicago in 1974 and is a life member and past president of the institute's alumni association. He is licensed to practice dentistry by the California State Board of Dental Examiners.

He was promoted to major general May 1, 1990, with same date of rank.
