Officials release Lackland ground accident report

An Air Combat Command accident investigation board report into the Jan. 12 death of a Combat Controller Selection Course student who was removed from life support three days after a training incident was released July 7 by Air Force officials here.

Based on the accident investigation board report, the initial event was a cardiac arrest caused by low oxygen to the heart that resulted in an irreversible brain injury as a result of a reduction in oxygen supply combined with reduced blood flow to the brain.

Staff Sgt. Kenneth Wilburn, assigned to an ACC base, was performing water skills training Jan. 9 at Lackland Air Force Base, Texas. Air Education and Training Command officials run the Combat Controller Selection Course.

Sergeant Wilburn lost consciousness while treading water at the base training pool. A Combat Controller Selection Course instructor in the water at the time immediately removed Sergeant Wilburn from the pool and the instructor and an emergency medical technician-qualified student began cardiopulmonary resuscitation. Another student called 911 and a nearby ambulance arrived within three minutes. Sergeant Wilburn was then transported to the on-base emergency department at Wilford Hall Medical Center where lifesaving efforts continued. Sergeant Wilburn never regained consciousness and was removed from life support Jan. 12.
